CMS Care Compare shows Providence Saint John's Health Center operating with a 30-day heart failure readmission rate materially above the national median, and well above Summit Cardiac's sub-8% program target. As part of the Providence health system — whose network already runs an established home-health and telehealth footprint across Southern California — HRRP penalty exposure scales directly with every preventable bounce-back across the HF, AMI, and CABG service lines. Providence's existing telehealth/RPM stack is strong; the gap is the in-person, ICU-credentialed layer that catches the decompensating patient before the bounce-back occurs.

Summit Cardiac's Augmentation

In-person, ICU/CCU-credentialed RN visits as augmentation to Providence Saint John's existing telehealth/RPM stack — Victor Musoke personally conducts every home visit in the Pacific Palisades, Brentwood, and Santa Monica neighborhoods where Providence Saint John's discharged cardiac patients return. Hands-on decompensation assessment that telehealth scales cannot replicate for the highest-acuity subset of the panel.
Hands-on assessment + escalation to the discharging Providence Saint John's cardiologist — when a patient shows signs of decompensation at home, the Summit team escalates directly to the discharging Providence Saint John's cardiologist (named end point on the care plan). Providence Saint John's retains clinical ownership; Summit provides the in-home presence and assessment bandwidth between visits.
Continuous monitoring stack — Masimo SpO2, KardiaMobile 6L ECG, and daily weights — that augments Providence's existing RPM stack rather than competes with it. Early-warning data streams to Providence Saint John's care team via the same channels they already monitor, with Summit's nurses serving as the on-the-ground response arm.
