CMS Care Compare reports UCLA Health's 30-day heart failure readmission rate at approximately 22%, materially above the national median and well above Summit Cardiac's sub-8% program target. Under the Hospital Readmissions Reduction Program, excess HF readmissions translate directly into Medicare payment reductions — a recurring annual penalty that compounds across the heart failure, AMI, and CABG service lines. UCLA already operates a strong telehealth and remote-patient-monitoring (RPM) program; the gap is the in-person, ICU-credentialed layer that catches the decompensating patient before the bounce-back occurs.
